Personal Accident Insurance - Claim Process

​​As soon as a claim occurs, please intimate immediately to our Help line number +91 22 48903009 (Paid)​, our call centre executive will be happy to help you with the claim procedure.
You need to furnish the following details when intimating your claim:

  • Your contact numbers
  • Policy number
  • Name of insured person who is injured
  • Date and time of accident
  • Location of loss
  • Brief description on how the accident took place
  • Extent of loss
  • Place and contact details of the Insured Person

Also...

  • Consult the doctor for precaution and treatment if required
  • Alert police authorities in case of road Traffic Accident
  • Notify your claim at our call center at the earliest

D​ocument Checklist for Claim Submission-

Accidental Death Claim Document Type:

  • Duly filled and signed Claim form
  • Original/Attested copy of Death Certificate
  • Attested copy of Post Mortem Examination report
  • In Case of Accident- Copy of Medico Level Certificate from hospital
  • Copy of Photo ID proof of Insured person(Employee/Member ID card)
  • Attested copy of FIR of local police station or Detailed Police Information note or Inquest Panchnama / Spot Panchnama (if applicable)
  • Original Cancelled Cheque in CTS 2010 format (Printed account number, IFSC code, Printed name) Mandatory. In case the name is not printed on cheque leaf, scanned copy of 1st page of passbook or the authorized bank statement.
  • As per IRDAI any claimed amount above one Lac, copy of PAN card/form 60 along with photo ID proof of the Claimant is mandatory and for below one Lac Photo identity proof (for eg.- Driving license, Election card, Passport etc.) is mandatory

Accidental Injury Claim Document Type:

  • PTD (Permanent Total Disability) & PPD (Permanent Partial Disability)
    • Duly filled and signed Claim form
    • Disability certificate issued by Senior Medical Officer mentioning the disability percentage.
    • Complete treatment record like Discharge summary, Consultation papers with supporting Investigation reports like X-ray/MRI etc.
    • In Case of Accident- Copy of Medico Level Certificate from hospital
    • Attested copy of FIR of local police station or Detailed Police Information note or Inquest Panchnama / Spot Panchnama (if applicable)
    • Coloured and clear photograph of Disabled person showing the disability
    • Income proof like Pay slips/Salary slips prior to the Date of loss.
    • Employee/Member Photo ID proof
    • Original Cancelled Cheque in CTS 2010 format (Printed account number, IFSC code, Printed name) Mandatory. In case the name is not printed on cheque leaf, scanned copy of 1st page of passbook or the authorized bank statement.
    • As per IRDAI any claimed amount above one Lac, copy of PAN card/form 60 along with photo ID proof of the Claimant is mandatory and for below one Lac Photo identity proof (for eg.- Driving license, Election card, Passport etc.) is mandatory
  • TTD (Temporary Total Disability):
    • Duly filled and signed Claim form
    • Medical Certificate confirming the Disability period and the probable date to resume duty/service
    • Complete treatment record like Discharge summary, Consultation papers with supporting Investigation reports like X-ray/MRI etc.
    • In Case of Accident- Copy of Medico Level Certificate from hospital
    • Attested copy of FIR of local police station or Detailed Police Information note or Inquest Panchnama / Spot Panchnama (if applicable)
    • Leave Certificate from the Employer mentioning the leave dates
    • Income proof like Pay slips/Salary slips prior to the Date of loss
    • Employee/Member Photo ID proof
    • Original Cancelled Cheque in CTS 2010 format (Printed account number, IFSC code, Printed name) Mandatory. In case the name is not printed on cheque leaf, scanned copy of 1st page of passbook or the authorized bank statement
