MISCELLENOUS CLAIMS

As soon as a claim occurs, please intimate immediately to our Regional Office. Our claims officials will be happy to help you with the claim procedure. Details of Regional offices can be found in Reliance General Insurance Office locator.

Following information needs to be furnished by you while intimating a claim:
  • Your Contact Numbers
  • Policy Number
  • Name of Insured person,
  • Date & Time of accident,
  • Location of Loss,
  • Brief description on how the accident took place,
  • Extent of loss
  • Place & contact details of the Insured Person (in case person intimating the claim is other than the insured person.

BAGGAGE INSURANCE CLAIM

IMMEDIATE ACTION AFTER LOSS:
  • You should inform the Police authorities immediately with a formal written complaint and obtain a copy of the FIR (First Information Report) which has to be given to the insurance company. This is a very important document required for processing your claim.
  • Intimate the insurance company with a rough estimate of loss as early as possible and obtain your claim form, before 14 days of the loss.
  • Please take all possible measures to minimise his loss.

CLAIMS PROCEDURE

Basic Documents:
  • The Police Report:It should include copies of your written complaint to the police at the nearest railway station/bus depot/airport and the FIRST INFORMATION REPORT (FIR).
  • Claim form: Duly filled and completed in all respects.
  • Claim Bill: Detailed claim bill with necessary bills and vouchers.
  • Documentary evidences about the value of property stolen through invoice, bill, books of account etc.
  • Final Survey Report: As made by the sureyor.
  • Copy of your Baggage Insurance policy certificate.
  • Letter of undertaking: You should provide a letter of authority in favour of the insurance company if there is any right of recovery.

BURGLARY INSURANCE CLAIM

IMMEDIATE ACTION AFTER LOSS:
  • You should inform the Police authorities immediately with a formal written complaint and obtain a copy of the FIR (First Information Report) which has to be given to the insurance company. This is a very important document required for processing your claim.
  • Intimate the insurance company with a rough estimate of loss as early as possible and obtain your claim form.
  • The insurance company will not be liable for any loss or damage, if the loss is not intimated to them within 14 days.
  • In case of small claims upto 2500/- we directly settles the claim on the basis of your completed 'claim form' and the 'Police Report'. For claims above Rs. 2500/- a surveyor is appointed by the insurance company to investigate your claim.

WORKMEN'S COMPENSATION INSURANCE CLAIM

IMMEDIATE ACTION AFTER ACCIDENT:
  • Give immediate written notice to the insurance company about the death or disablement of the employer.
  • Intimation of Accident to Factory Inspector.
  • Furnish all information and documentary evidence as the insurance company may require.
  • Deposit the compensation payable to the employee with Workmen Compensation Commissioner. Obtain their receipt and submit the same to Insurance Company.
  • If any notice is received from W.C. Commissioner, send the same to Insurance Company.
  • To collect the relevant data with the objective to quantify reasonable and just compensation, a trained investigator is appointed by the insurance company.
  • At the same time you should be submit claim form along with relevant documents.

CLAIMS PROCEDURE


Basic Documents:


1. General for all type of claims:

  • Claim Form duly filled in & signed.
  • Claim Bill.

2. Temporary Disablement Claims:

  • Medical Certificate regarding Cause & Duration of Disablement.
  • Medical Bills.

3. Permanent Disablement Claims:

  • Medical Certificate regarding Disablement.
  • Memorandum of Agreement as per W.C Act between Insured and the injured workman.

4. Fatal Claims:

  • Death certificate.
  • Copy of post Mortem report.
  • F.I.R / Final Investigation report
  • Form A of W.C Act duly completed by the Insured.
  • Statement of Witnesses ,if any

FIDELITY GUARANTEE INSURANCE CLAIM

IMMEDIATE ACTION AFTER LOSS:
  • You should inform the Police authorities immediately with a formal written complaint and obtain a copy of the FIR (First Information Report) which has to be given to the insurance company. This is a very important document required for processing your claim.
  • Intimate the insurance company with a rough estimate of loss as early as possible.
  • The insurance company will not be liable for any loss or damage, if the loss is not intimated to them within 14 days.
  • Obtain the Claim form, from the insurance company and fill in the details as required.
  • Submit all relevant documents and details to insurance company within 3 months after giving notice of loss.
  • Take immediate steps against the defaulting employee for the recovery of cash/ goods as the case may be and also other disciplinary action required, depending on the case.
  • Establish the act of infidelity committed by the particular employee covered under the policy.
  • In case of small claims upto 2500/- we directly settles the claim on the basis of your completed 'claim form' and the 'Police Report'. For claims above Rs. 2500/- a surveyor is appointed by the insurance company to investigate your claim.

CLAIMS PROCEDURE

Basic Documents:
  • The Police Report: It should include copies of your written complaint to the police and the FIRST AND FINAL INVESTIGATION REPORT (FIR).
  • Claim form: Duly filled and completed in all respects.
  • Not to release any payment to the defaulting employee without insurance company's consent.
  • Conduct Departmental inquiry against the defaulting employee (where required as per companies rules)
  • Claim Bill: Detailed claim bill with necessary bills and vouchers.
  • Documentary evidences about the value of property stolen through invoice, bill, books of account etc.
  • Letter of Indemnity: You should provide a letter of authority in favour of the insurance company if there is any right of recovery.
  • The Insurance Company will make final payment of claim, shall deduct such amount like wages, bonus, incentives, allowances which the policyholder has to pay to the employee committing fraud, since these amount can be recovered by the employer i.e. policyholder.

Claim form:

On receipt of notice of loss, the insurance company will issue a claim form. The following information will be required to be given, to process the claim.
  • Details about the employee responsible for loss, eg name, address, designation, nature of duties etc.
  • Details about loss, e.g. The date of discovery, mode of operation, etc.
  • Action taken against the employee
  • Details of amount of loss e.g. extent of loss as discovered from the books of accounts, direct recoveries, if any, made from the defaulting employee, amounts due to the employee, cash security given by the employee etc.
  • Details of immovable property owned by the employees if known
  • Details of other insurance, if any.

LIABILITY INSURANCE CLAIMS

IMMEDIATE ACTION AFTER LOSS :

  • Intimation of any event which may give rise to any claim under the policy is to be informed to insurance company giving a brief detail about the period of accident.
  • The Insurance Company, on receipt of a notice, will issue a claim form for ascertaining the details of accident and injury or damage caused to the third parties. The following questions will be asked in the Claim Form.
    • Details of Policyholder : Name, address and occupation, the policy number, period of insurance, the limits of indemnity in respect of anyone accident and for whole period, the place where accident occurred and whether it is within the scope of policy; and persons responsible for causing the injury or damage and whether they are covered within the policy. These details are necessary by the insurance company in order to find out whether the claim is prima facie admissible in terms of the policy.
    • Details about the Accident: The insured has to give a brief account of as to how the injury/or damage was caused. If the negligence of any employee is involved, explain in details, how and why the accident has occurred.
    • Details about Third Parties : A brief details such as name, address, age, status, occupation, earnings and nature of injury caused to third parties.
    • Amount of Claim : You will have to give particulars of the amount of claim made by the aggrieved persons.
  • Do not attempt or offer or compromise any settlement with the third parties, without the knowledge and consent of the insurance company.
  • If the notice or summons are received directly from the court, inform the insurance company and competent advocate should be appointed.
  • No claim is payable under the policy unless the cause of action arises in India and the liability to pay claim is established against the Insured in an Indian Court. It is also to be understood that only Indian Law shall be applicable in such action.

CLAIMS PROCEDURE


Basic Documents

  • The Police Report :It should include copies of your written complaint to the police and the FIRST AND FINAL INVESTIGATION REPORT (FIR).
  • Claim form : Duly filled and completed in all respects .
  • When appointed by Insurance company, co-operate with the investigator.
  • If any notice of claim is received the same should be sent to the Insurance company without admitting any liability.
  • When the matter is referred to the court co-operate with the Insurance company in giving information to the lawyer and also arrange for witnesses to defend the case as no insurance cover exists.
  • In case of death, submit post-mortem certificate.
  • In case of injuries proper medical certificates and full set of bills towards medical expenses.
  • In case of Property damage, details of ownership and the owner's financial interest in the property and the extent of damage with certificates from appropriate authorities like the Gazette Officer or a Magistrate or a Notary Public or a Village Administrative.

MONEY INSURANCE CLAIM

IMMEDIATE ACTION AFTER LOSS :

  • Inform the police station and submit a formal written complaint to obtain a FIR (First Information Report), a copy of which has to be given to the insurance company. This is a very important document required for processing your claim.
  • Intimate the insurance company together with a rough estimate of loss as early as possible. The insurance company will not be liable for any loss or damage, if the same is not intimated to them within 14 days.
  • Obtain the Claim form and fill in the details as required by the insurance company.
  • Take steps to discover the guilty person to recover the lost cash.
  • Furnish vouchers / proofs of ownership and other evidence to substantiate the claim.
  • If the dishonesty of the employee is involved, departmental proceedings or criminal action should be initiated against the erring employee.
